Artificial sweeteners: what the WHO's 2023 ruling actually said (and didn't)

Aspartame made global headlines when the WHO classified it as 'possibly carcinogenic' — the same category as pickled vegetables and aloe vera. The nuance behind that ruling matters more than the headline, and so does what sweeteners do to appetite and the gut.

In 2023 the World Health Organization's cancer research arm classified aspartame as "possibly carcinogenic to humans", and the headlines wrote themselves. What most coverage left out is that this is the WHO's weakest evidence category — Group 2B — which also contains pickled vegetables, aloe vera extract and working as a carpenter. The ruling reflects limited evidence, not a confirmed risk, and it is worth separating from the more genuinely useful open questions about sweeteners.

What the classification actually means

IARC's Group 2B means 'limited evidence in humans, less than sufficient in animals' — a category for substances where a link cannot be ruled out but is far from established. In the same WHO review, the Joint FAO/WHO expert committee on food additives (JECFA) reaffirmed the existing acceptable daily intake for aspartame — around 40 mg/kg bodyweight, roughly 12–14 cans of diet soda a day for most adults — as still safe. The two bodies published different messages from the same evidence review, which is exactly why the headline outran the nuance.

The more interesting open questions

Separate from the cancer question, two areas of active research are arguably more relevant day to day. Suez and colleagues (2014, Nature; follow-up 2022, Cell) found some non-nutritive sweeteners altered gut microbiome composition and glucose tolerance in a subset of people — an individualised, not universal, effect. And there is an ongoing debate about whether very sweet-tasting foods, even calorie-free, can maintain a preference for sweetness or, in some studies, blunt the normal satiety signal tied to sweet taste and calories arriving together.

Where they clearly help

None of this erases the strongest use case: replacing sugar-sweetened drinks with diet versions reliably cuts calorie and sugar intake, and for someone drinking multiple sugary sodas a day, that swap is close to unambiguously positive — especially against the sodium, added-sugar and glycaemic levers covered elsewhere in this guide.
