Protecting immune function as you age: what actually moves the needle, according to a controlled trial

A randomised trial found that months of cardiovascular exercise training extended how long older adults stayed protected after a flu vaccine — one of the few controlled human trials showing exercise can measurably improve an aging immune system’s real-world performance.

Immunosenescence — the gradual decline of immune function with age — is a major reason older adults are both more susceptible to infection and less protected by vaccines. Most of the popular discussion around it is theoretical; a randomised trial testing whether exercise training could actually change a real-world immune outcome is much rarer.

What the trial found

Researchers randomised 144 sedentary older adults to a supervised cardiovascular exercise-training program or a non-exercise control group, then vaccinated all participants against influenza and tracked antibody levels over time. The exercise-training group maintained protective antibody levels (seroprotection) for a significantly longer duration after vaccination than the control group — a directly measurable, clinically relevant immune outcome, not just a lab marker.

The broader immunosenescence picture

A more recent 2026 review frames this kind of finding under the concept of "immunofitness" — the idea that vaccination together with lifestyle factors like exercise, sleep and nutrition combine to counter both immunosenescence (declining immune defence) and inflammaging (chronic low-grade inflammation), rather than either factor working in isolation.

The caveats worth noting

The core trial here is now over 15 years old and moderately sized (144 participants) — a genuinely important and still-cited result, but one that deserves larger modern replication before being treated as definitive. Exercise is best understood as an adjunct that improves the immune system’s response to vaccination, not a substitute for the vaccine itself.
