Plant-based done right: the four nutrients to watch (and how to verify them)
A well-planned plant-based diet is one of the healthiest ways to eat. 'Well-planned' is doing a lot of work in that sentence — and it comes down to four nutrients that need active attention and, ideally, a blood test to confirm.

But 'plant-based' is not automatically 'nutritionally complete' — a handful of nutrients are either absent from plants or poorly absorbed from them, and ignoring them is how an otherwise excellent diet quietly creates a deficiency. Four deserve active management.
1. Vitamin B12 — non-negotiable
B12 is made by bacteria, not plants, so a diet with no animal products contains essentially none. Pawlak and colleagues (2013, Nutrition Reviews) found deficiency common in vegans not supplementing. This is not optional to manage: B12 supports nerve function and red blood cell formation, and prolonged deficiency causes irreversible damage. A supplement or reliably fortified foods, plus a periodic serum B12 check (target above 400 pg/mL), is the standard of care.
2. Iron — present, but harder to absorb
Plants contain non-heme iron, absorbed far less efficiently than the heme iron in meat, and inhibited by the phytates and polyphenols in the same plant foods. Hallberg (1989) showed vitamin C can multiply non-heme absorption two- to three-fold — so pairing iron-rich plants with citrus or peppers, and keeping tea and coffee away from those meals, is the practical lever. Confirm with ferritin, not symptoms.
Plant iron is not missing — it is just harder to absorb. Vitamin C at the same meal is the difference-maker.
3. Omega-3 EPA and DHA — the conversion tax
Flax, chia and walnuts provide ALA, but the body converts ALA to the active EPA and DHA at low rates — often under 10% (Burdge, 2006). Algae-derived EPA/DHA supplements are the reliable vegan route, and an Omega-3 Index test tells you objectively whether your intake is enough.
4. Protein quality — combine, don't stress
Most single plant proteins are lower in one or more essential amino acids and less leucine-dense, giving them lower DIAAS scores than animal proteins. The fix is not obsessive meal-by-meal combining but eating a variety of protein sources across the day — legumes, grains, soy, nuts — and slightly larger portions to clear the leucine threshold.
